SMS Terms & Opt-In Policy

This SMS Terms & Opt-In Policy describes how FORMINO (a brand of KS LUX GROUP INC, "FORMINO," "we," "us," or "our") sends text messages to users who voluntarily opt in, the purpose of those messages, and how to stop receiving them.

1. Program Description

FORMINO operates a transactional SMS program that delivers one-time verification codes to professional account holders (contractors, designers, fabricators, and retailers) registering on our partner portal at portal.formino.us. These messages are used strictly to verify that the phone number provided during signup belongs to the user creating the account.

2. How to Opt In

Users opt in to the SMS program during account registration on the FORMINO partner portal. The opt-in flow is as follows:

  1. Navigate to https://portal.formino.us/auth#sign-up and select the Sign Up tab.
  2. Choose an account type (Contractor, Designer/Architect, Retailer, or Fabricator).
  3. Enter your full name, company name, and mobile phone number.
  4. Read the SMS consent disclosure and check the box labeled: "I agree to receive a one-time SMS verification code from Formino at the phone number provided. Standard message and data rates may apply. Reply STOP to opt out or HELP for help."
  5. Submit the form. A one-time 6-digit verification code is sent to the phone number provided.
  6. Enter the verification code to complete account creation.

Consent to receive the SMS verification code is not a condition of purchase. Users may decline by leaving the phone number field blank and completing account creation via email only.

3. Message Types & Frequency

  • Message type: One-time password (OTP) for phone number verification at the time of account creation.
  • Frequency: One message per signup attempt. A user may request a resend if the original code is not received.
  • Sender: FORMINO via Twilio.

4. Example Message

"Your FORMINO verification code is 123456. Reply STOP to opt out, HELP for help. Msg & data rates may apply."

5. Opt-Out Instructions

You can opt out of the SMS program at any time by replying STOP to any message received from FORMINO. After replying STOP, you will receive a single confirmation message and will not receive further SMS messages from this program.

7. Message & Data Rates

Message and data rates may apply, depending on your mobile carrier and plan. FORMINO does not charge a fee for these messages. Contact your mobile carrier for details on your messaging plan.

8. Supported Carriers

The program is supported on all major U.S. mobile carriers, including AT&T, T-Mobile, Verizon, Sprint, U.S. Cellular, and Boost. Carriers are not liable for delayed or undelivered messages.

9. Privacy

Phone numbers collected through this program are used solely for the purpose of SMS verification and account security. 10. Changes to This Policy

We may update this SMS Terms & Opt-In Policy from time to time. The effective date at the top of this page reflects the most recent revision. Continued use of the SMS program after changes are posted constitutes your acceptance of the revised terms.
